Corrections to Personal Information

Routine changes or corrections to personal information such as a new address or a new telephone number, do not require a formal request under ATIA. Please contact the school or the department that has the information and inform them of the change or correction. You may be asked to provide additional documentation to support the change or correction.

If you believe the information that the CSCN has about you is incorrect, you can make a request under ATIA to have your personal information corrected. If the CSCN does not agree to change the information, an annotation will be placed on the file where the personal information resides, indicating that you have requested the change.

Please be aware that opinions, including professional or expert opinions, cannot be corrected.

A request under ATIA to correct personal information must be made in writing. You may submit your request for correction by email to This email address is being protected from spambots. You need JavaScript enabled to view it. or by mail to:

 When to Expect a Response

ATIA requires that the CSCN respond within 30 business days of receiving a request. The response will either provide the requested information or explain why the information is not being disclosed. In the case of a request for correction of personal information, the response will either state that the correction has been made or advise that the record has not been corrected, but it has been annotated. The 30-day deadline may be extended if, for example large volumes of records are involved, enough detail to identify the record(s) hasn’t been provided, or third parties need to be consulted.
